I {Client Name}, hereby acknowledge and agree to the following terms and conditions before participating in any activities, classes, or programs offered by Body Core Studios LLC: Assumption of Risk: I understand and acknowledge that participating in high-intensity Pilates activities involves inherent risks, including but not limited to, bodily injury, strain, or other harm. I voluntarily assume all risks associated with participation, including any risks arising from negligence or carelessness on the part of Body Core Studios LLC, its instructors, or employees. Physical Health: I certify that I am physically fit and have no medical conditions that would prevent me from participating in high-intensity Pilates activities. I agree to inform Body Core Studios LLC of any changes to my health or medical condition that may affect my ability to participate safely. Pregnancy: I understand that certain high-intensity Pilates exercises may not be suitable during pregnancy. I acknowledge that it is my responsibility to inform the instructor if I am pregnant or become pregnant during the course of my participation in high-intensity Pilates activities. I understand that Body Core Studios LLC recommends consulting with a healthcare professional before participating in high-intensity Pilates (or any participating in any of studio services) during pregnancy and that I participate at my own risk. Release of Liability: In consideration of being allowed to participate in high-intensity Pilates activities (or any Body Core Studios services), I hereby release, waive, discharge, and covenant not to sue Body Core Studios LLC, its owners, instructors, employees, or agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, whether caused by negligence or otherwise. Indemnification: I agree to indemnify and hold harmless Body Core Studios LLC, its owners, instructors, employees, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to my participation in high-intensity Pilates activities, including but not limited to, any claims made by third parties. Photo/Video Release: I consent to the use of any photographs, videos, or other media taken during high-intensity Pilates activities (or any Body Core Studios classes and services) by Body Core Studios LLC for promotional or educational purposes. I have read this liability waiver form and fully understand its contents. I am aware that by signing this form, I am waiving certain legal rights, including the right to sue. I voluntarily agree to be bound by its terms.
